เเพทย์สั่งจ่าย Nicardipine IV (10mg/10ml) โดยให้ 20 mg + NSS 100 ml (1:5) IV rate start 15 ml/hr ซึ่งก่อนหน้านี้ผู้ป่วยได้รับ Acetar อยู่ สามารถให้ร่วมกับผ่าน T way ได้มั้ย หรือต้องเเยกเส้น มีอะไรที่ต้อง monitor เพิ่มเติมบ้าง

คำตอบ

Nicardipine เป็นยากลุ่ม calcium channel blockers (CCBs) ยานี้ไม่สามารถผสมร่วมกับ sodium bicarbonate และ lactate ringer’s solution ส่วน Acetar หรือ acetated ringer’s injection นั้นไม่มีหลักฐานว่ามีความคงตัวหากบริหารร่วมกันผ่านทาง y-site injection ดังนั้นหากเป็นไปได้ควรบริหารยาทั้งสองชนิดแยกกัน แต่หากจำเป็นต้องให้ร่วมกัน ควรตรวจสอบการเปลี่ยนสภาพของยาในสายยาง และติดตามค่าสัญญาณชีพอย่างใกล้ชิด
