สอบถามค่ะ ในปัจจุบัน ยาที่มี drug interaction กับ fluoxetine เมื่อใช้ร่วมกัน แล้วทำให้เกิด serotonin syndrome มียาตัวไหนบ้างคะ

ยาหลายชนิดสามารถเพิ่มระดับ serotonin เช่น ยากลุ่ม monoamine oxidase inhibitors (MAOIs: phenelzine, selegiline) ยากลุ่ม tricyclic antidepressants (TCAs: amitriptyline, nortriptyline) ยากลุ่ม triptans สำหรับรักษาอาการปวดศีรษะไมเกรน (lithium) ยาแก้ปวด (tramadol) ยากลุ่ม selective serotonin reuptake inhibitors (SSRIs: sertraline, citalopram, escitalopram) และ serotonin and norepinephrine reuptake inhibitors (SNRIs: duloxetine) รวมไปถึงสมุนไพร เช่น St. John’s Wort ซึ่งการใช้ fluoxetine ร่วมกับยาหรือสมุนไพรเหล่านี้สามารถเพิ่มความเสี่ยงการเกิด serotonin syndrome ได้เช่นกัน โดยเฉพาะในกรณีที่รับประทานยาในขนาดสูงหรือมีปฏิกิริยาระหว่างยา (drug interaction) ที่เพิ่มระดับ serotonin ข้างต้น ดังนั้นหากมีการรับประทานยาเหล่านี้อยู่ควรตรวจสอบรายการยาเดิมหรือยาที่จะได้รับใหม่ หรือปรึกษาแพทย์-เภสัชกรเพื่อประเมินความเหมาะสมของการใช้ยาและป้องกันการเกิด drug interaction และ serotonin syndrome จากยาเหล่านี้ [1,2]
