การให้ยา Phenytoin inj. IV loading 750 mg ตามด้วย IV 100 mg ทุก 8 ชม. อยากทราบว่าระยะห่างการให้ยา loading 750 mg กับ maintenance 100 mg ต้องห่างกันกี่ชั่วโมงครับ จำเป็นต้องเป็น 24 ชั่วโมงหรือไม่

คำตอบ

Phenytoin ในรูปแบบยาฉีดทางหลอดเลือดดำ (intravenous administration) มีวิธีการบริหารยา คือ loading dose 10-15 mg/kg จากนั้นตามด้วย maintenance dose 100 mg ทุก 6-8 ชั่วโมง และมีอัตราการบริหารยาไม่เกิน 50 mg/min นอกจากนี้ phenytoin เป็นยาที่มีดัชนีการรักษาแคบ (narrow therapeutic index) กล่าวคือ ยามีระดับความเข้มข้นในเลือดที่ให้ผลการรักษา (therapeutic level) ใกล้เคียงกับระดับความเข้มข้นที่ทำให้เกิดพิษ (toxic level) ทำให้การให้ยาซ้ำในช่วงเวลาที่สั้นเกินไปอาจส่งผลให้ระดับยาในเลือดสูงเกินช่วงที่ปลอดภัยได้ จึงไม่แนะนำให้ใช้สูตรคำนวณระยะเวลาการให้ยาแบบทั่วไป (เช่น halfway rule) โดยระยะเวลาการเริ่ม maintenance dose มีคำแนะนำที่แตกต่างกันในหลายแหล่งข้อมูลโดย UpToDate แนะนำให้เริ่ม maintenance dose ประมาณ 8–12 ชั่วโมงหลัง loading dose (เอกสารอ้างอิง 2) ขณะที่ Adult Therapeutic Handbook แนะนำ 12–24 ชั่วโมงหลัง loading dose (เอกสารอ้างอิง 3) ทั้งนี้สามารถพิจารณาเลือกช่วงเวลาที่เหมาะสมตามบริบททางคลินิกและความเสี่ยงต่อการเกิดพิษของผู้ป่วยแต่ละราย รวมถึงการวัดระดับยาในเลือดจะช่วยให้สามารถเลือกช่วงเวลาได้เหมาะสมมากที่สุด
