Buscopan หรือ hyoscine butylbromide เป็นยารับประทานที่มีฤทธิ์ต้านการหดเกร็งของกล้ามเนื้อเรียบ มีข้อบ่งใช้สำหรับรักษาอาการปวดท้องที่สัมพันธ์กับอาการปวดบิด โดยสามารถใช้ได้อย่างปลอดภัยในผู้ป่วยโรคไตเรื้อรัง รวมถึงผู้ป่วยที่มีค่าอัตราการกรองของไต (estimated glomerular filtration rate; eGFR) ต่ำกว่า 30 mL/min/1.73 m2 โดยไม่ต้องปรับขนาดยา เนื่องจาก hyoscine ในรูปของเกลือ butylbromide ถูกดูดซึมผ่านทางเดินอาหารเข้าสู่กระแสเลือดต่ำมาก โดยมีค่า oral bioavailability < 1% จึงมีปริมาณยาที่ขับออกทางไตน้อยมาก โดยการรับประทาน hyoscine butylbromide ขนาดสูง 500 mg พบปริมาณยาในปัสสาวะ 0.16%
Meteospasmyl เป็นยาสูตรผสมระหว่าง alverine citrate ขนาด 60 mg ซึ่งเป็นยาต้านการหดเกร็งของกล้ามเนื้อเรียบและ simethicone ขนาด 300 mg ซึ่งเป็นยาขับลม โดย simethicone มีความปลอดภัยในผู้ป่วยโรคไตเรื้อรังทุกระดับ เนื่องจากไม่ถูกดูดซึมเข้าสู่กระแสเลือดและขับออกทางอุจจาระในรูปเดิม ในขณะที่ alverine citrate ถูกเปลี่ยนสภาพอย่างรวดเร็วที่ตับไปเป็น primary metabolite ที่สามารถออกฤทธิ์ได้ และจากนั้นจะถูกเปลี่ยนสภาพไปเป็น secondary metabolites 2 ชนิด ซึ่งไม่มีฤทธิ์ทางยา โดย metabolites ทั้งหมดขับออกทางไต อย่างไรก็ตามปัจจุบันยังไม่พบข้อมูลความปลอดภัยของการใช้ alverine citrate ในผู้ป่วยโรคไตเรื้อรัง
ดังนั้นหากเปรียบเทียบกันระหว่างยาทั้งคู่ การใช้ Buscopan อาจเป็นตัวเลือกสำหรับรักษาอาการปวดเกร็งช่องท้องในผู้ป่วยโรคไตเรื้อรังที่มีข้อมูลสนับสนุนที่ชัดเจนกว่า
