Alverine citrate + Simeticone 300 mg มีที่ใช้ใน bloating เฉยๆหรือไม่ 1.เนื่องจากไปฟังแล้วมีคนบอกว่า ใช้รักษา bloating ได้เพราะมีsimethicone dose สูงกว่า ยาอื่นๆ ที่มีตามท้องตลาด แต่ได้สอบถามไปแล้วว่ามี alverine แบบนี้ จะได้ alverine ไปโดยไม่จำเป็นหรือไม่ จึงอยากทราบคำตอบว่า ยาตัวนี้ มีที่ใช้ใน bloating เฉยๆ โดยไม่มีอาการปวดท้องจริงหรือไม่ครับ 2.dose simethicone relate กับการแก้อาการที่เยอะขึ้นหรือไม่ แล้ว dose ที่แนะนำใน bloating คือเท่าใด

คำตอบ

ยาสูตรผสมระหว่าง alverine citrate 60 mg และ simeticone 300 mg ที่นิยมใช้ในประเทศไทย คือ Meteospasmyl® ซึ่งมีข้อบ่งใช้ในการลดอาการปวดเกร็ง (abdominal cramp/pain) จากฤทธิ์ของ alverine ร่วมกับมีอาการท้องอืดหรือมีแก๊สในกระเพาะอาหาร (bloating) จากฤทธิ์ของ simeticone ดังนั้นจึงเหมาะสำหรับผู้ที่มีอาการปวดเกร็งร่วมด้วยในผู้ป่วยโรคลำไส้แปรปรวน (irritable bowel syndrome; IBS) หากผู้ป่วยมีอาการท้องอืดเพียงอย่างเดียว อาจพิจารณาใช้ยา simeticone เดี่ยวเป็นทางเลือกแรก โดยขนาดยา simeticone เดี่ยวสำหรับลดอาการท้องอืด คือ 40 – 125 mg หลังอาหาร 3 เวลาและก่อนนอน และไม่เกิน 500 mg ต่อวัน อย่างไรก็ตามหากต้องการใช้ Meteospasmyl® ซึ่งเอกสารกำกับยาแนะนำให้รับประทานครั้งละ 1 เม็ด วันละ 2-3 ครั้ง จะทำให้ได้รับ simeticone ปริมาณ 600-900 mg แม้ว่าตัวยา simeticone ไม่ถูกดูดซึมเข้าสู่กระแสเลือดและออกฤทธิ์เฉพาะทางเดินอาหาร รวมถึงไม่มีรายงานการเกิดอาการไม่พึงประสงค์รุนแรง แต่ปัจจุบันยังไม่พบหลักฐานที่ชัดเจนเกี่ยวกับความสัมพันธ์ระหว่างขนาดยา simethicone กับผลลัพธ์การรักษาอาการท้องอืด และการใช้ขนาดยาที่เพิ่มสูงขึ้นไม่ได้แสดงถึงผลการออกฤทธิ์ที่เพิ่มขึ้นอย่างเป็นสัดส่วน เนื่องด้วยกลไกการออกฤทธิ์ของยาที่ออกฤทธิ์ลดแรงตึงผิวของฟองแก๊สช่วยให้แก๊สถูกขับออกง่ายขึ้น ไม่ใช่ลดการผลิตแก๊ส รวมทั้งการใช้ยาสูตรผสมนี้ยังอาจทำให้ผู้ป่วยได้รับ alverine ไปพร้อมกันด้วย ซึ่งการได้รับยา alverine ไม่ได้ให้ประโยชน์เพิ่มเติมอย่างชัดเจนในอาการท้องอืด รวมถึงอาจเพิ่มความเสี่ยงต่อการเกิดอาการไม่พึงประสงค์จากยาและเพิ่มค่าใช้จ่ายในการรักษาเมื่อเทียบกับการใช้ยา simeticone เดี่ยวซึ่งมีราคาถูกกว่าและเพียงพอสำหรับอาการท้องอืดทั่วไป
