ยา Harnal ถ้าใช้ร่วมกับยา rifampicin, INH, ethambutol, wafarin ได้หรือไม่?

ยา Harnal? มีตัวยาสำคัญ คือ tamsulosin เป็น selective alpha-1, antagonist ซึ่ง metabolize ผ่าน enz cytochrome P450 oxidase (cyp) ในตับส่วนใหญ่เป็น cyp3A4 และ cyp2D6 isozyme ยาที่อาจเกิดปฏิกิริยาระหว่างกันได้แก่ INH (isoniazid), wafarin และ rifampin ซึ่งเป็น cyp3A4 inhibitor, substrate และ inducer ตามลำดับ อย่างไรก็ตาม มีเพียงยา wafarin ตัวเดียวที่มีการเตือนให้ใช้อย่างระวังอาจจะเพิ่มฤทธิ์ anticoagulant ของมันได้เมื่อใช้ร่วมกับ tamsulosin ขณะที่ยาตัวอื่นในกลุ่มเดียวกันได้แก่ doxazosin, prazosin, terazosin และ alfuzosin ไม่มีรายงานการเกิดปฏิกิริยาระหว่างยาดังกล่าว ดังนั้น หากใช้ tamsulosin ควรติดตามอาการเลือดออก (bleeding) จาก wafarin หรือไม่ก็เปลี่ยนไปใช้ยาตัวอื่นในกลุ่มเดียวกันแทน
