สงสัยว่า Tonsillitis กิน Amoxicillin 500 mg 1 เม็ดวันละ 3 ครั้ง เป็นเวลา 10 วันได้ไหมครับ หรือได้แค่ 1 เม็ดวันละ 2 ครั้ง

คำตอบ

กรณีต่อมทอนซิลอักเสบ (tonsillitis) จากเชื้อแบคทีเรีย Group A Streptococcus (GAS) ที่ได้รับการวินิจฉัยว่าเป็น GAS pharyngitis และไม่มีข้อห้ามใช้ penicillin แนวทางของ Centers for Disease Control and Prevention (CDC) และ Infectious Diseases Society of America (IDSA) แนะนำ amoxicillin ขนาด 50 mg/kg วันละครั้ง (สูงสุด 1,000 mg/วัน) เป็นเวลา 10 วัน หรือขนาด 25 mg/kg วันละ 2 ครั้ง (สูงสุด 500 mg/ครั้ง) เป็นเวลา 10 วัน ดังนั้นในผู้ใหญ่ ขนาด amoxicillin 500 mg วันละ 2 ครั้ง เป็นเวลา 10 วัน เป็นขนาดที่อยู่ในแนวทางแนะนำ

สำหรับ amoxicillin 500 mg วันละ 3 ครั้ง ไม่ใช่ขนาดมาตรฐานที่ CDC หรือ IDSA แนะนำสำหรับการรักษา GAS pharyngitis โดยตรง แต่เป็นการใช้สำหรับข้อบ่งใช้อื่น ๆ ดังนั้นหากเป็น GAS pharyngitis ไม่ควรปรับเป็นวันละ 3 ครั้งเองเพียงเพราะต้องการเพิ่มความถี่ในการรับประทานยา เนื่องจากขนาดยาและความถี่ของการใช้ยาขึ้นกับชนิดและตำแหน่งของการติดเชื้อที่มีข้อมูลการศึกษาและเป็นที่ยอมรับ

ทั้งนี้ tonsillitis ไม่ได้เกิดจากเชื้อแบคทีเรียเสมอไป โดย acute sore throat รวมถึง pharyngitis และ tonsillitis มักเกิดจากการติดเชื้อไวรัส และไม่ควรใช้ยาปฏิชีวนะรักษา หากมีอาการที่ชี้ไปทางไวรัส เช่น ไอ น้ำมูก เสียงแหบ หรือตาแดง ควรพิจารณาสาเหตุจากไวรัสก่อน หากเป็นเพียงอาการ “tonsillitis” โดยยังไม่ทราบว่าเกิดจาก GAS หรือไม่ ไม่ควรสรุปจากชื่อโรคเพียงอย่างเดียวว่าต้องใช้ amoxicillin ควรประเมินอาการและความเป็นไปได้ของ GAS ก่อน และหากมีข้อบ่งชี้ควรตรวจเพื่อยืนยันการติดเชื้อ
