แนวทางการรักษาภาวะโพแทสเซียมในเลือดสูง (hyperkalemia) โดยทั่วไป คือ sodium bicarbonate (NaHCO3) 50 mEq ทางหลอดเลือดดำ แบบ push ประมาณ 5 นาที และการให้กลูโคส 25 กรัม (50 mL ของ 50% Dextrose) ร่วมกับ regular insulin (RI) ขนาด 5–10 ยูนิต ทางหลอดเลือดดำ แบบ push ภายในประมาณ 15–30 นาที งานวิจัยในห้องฉุกเฉินของสหรัฐอเมริกาพบว่า การใช้อินซูลินขนาดต่ำ (5 ยูนิต) สามารถลดระดับโพแทสเซียมได้ใกล้เคียงกับขนาดมาตรฐาน (10 ยูนิต) และอาจพบภาวะน้ำตาลในเลือดต่ำได้น้อยกว่า อย่างไรก็ตามแนวทางเวชปฏิบัติส่วนใหญ่ยังคงใช้ขนาด 10 ยูนิตเป็นขนาดมาตรฐานในการรักษาภาวะโพแทสเซียมสูงรุนแรง ทั้งนี้ระยะเวลาและวิธีการให้ยาที่แน่นอนควรอ้างอิงตามแนวทางของแต่ละโรงพยาบาล คำสั่งแพทย์ และการประเมินสภาพผู้ป่วยเป็นรายบุคคล
