| บทคัดย่อ: โครงการพิเศษนี้เป็นการศึกษาเชิงคุณภาพเพื่อทำความเข้าใจเกี่ยวกับความรู้ การรับรู้ ความจำเป็น การรับรู้ความเสี่ยง ลักษณะการใช้ และเหตุผลในการใช้ยารับประทาน isotretinoin โดยทำการเลือกผู้ให้ข้อมูลด้วยวิธี purposive sampling และ snowball technique มาทำการ สัมภาษณ์โดยใช้แบบสัมภาษณ์กึ่งโครงสร้างที่เตรียมไว้ ผลจากการสัมภาษณ์ผู้ให้ข้อมูลจำนวน 20 คน ซึ่งมีอายุระหว่าง 15-30 ปี พบว่าผู้ให้ข้อมูลทั้งหมดเคยได้รับการรักษาอย่างอื่นมาก่อนการ ใช้ยารับประทาน isotretinoin แม้ว่าบางส่วนจะไม่ได้เป็นการรักษาตามแนวทางที่ระบุไว้ก่อนการ ใช้ยารับประทาน isotretinoin ผู้ให้ข้อมูลส่วนมากได้รับยาโดยแพทย์เป็นผู้พิจารณาสั่งจ่ายยา แต่มี ผู้ให้สัมภาษณ์บางราย (4 คน) ที่ซื้อยารับประทานเองโดยไม่มีใบสั่งแพทย์ โดย 1 คนเคยได้รับยา จากแพทย์จากนั้นจึงนำไปซื้อใช้เองจากร้านยา เมื่อสอบถามถึงความจำเป็นของการใช้ยา รับประทาน isotretinoin สามารถแบ่งได้เป็น ความจำเป็นทางการแพทย์เนื่องจากเป็นสิวอักเสบ รุนแรงพิจารณาโดยแพทย์ และความจำเป็นจากเหตุผลส่วนตัวของผู้ให้ข้อมูล เช่น ความกังวล เกี่ยวกับความสวยความงามของตนเอง ในเรื่องความคุ้มค่าในการใช้ ผู้ให้ข้อมูล 17 คนระบุว่ามี ความคุ้มค่าเนื่องจากให้ผลการรักษาที่มีประสิทธิภาพแม้ว่าจะมีราคาค่อนข้างสูง ด้านอาการ ข้างเคียงผู้ให้ข้อมูลทุกคนรับรู้ว่าการใช้ยาชนิดนี้จะทำให้ปากแห้ง มี10 คน ที่รับรู้ว่าสามารถทำให้ ทารกวิกลรูป อย่างไรก็ตามมีเพียง 5 คนที่รับรู้ว่ายานี้มีผลต่อการทำงานของตับ นอกจากไม่พบว่า มีรายใดเลยที่เคยได้รับการตรวจทางห้องปฏิบัติการเพื่อติดตามการทำงานของตับและระดับไขมัน จากการศึกษาในครั้งนี้เภสัชกรควรมีส่วนร่วมในการให้คำปรึกษาแนะนำเรื่องการใช้ยารับประทาน isotretinoinให้มากขึ้น โดยควรให้ความรู้เรื่องข้อบ่งใช้ อาการข้างเคียงรวมถึงการติดตามการใช้ยา เพื่อให้ผู้ที่ใช้ยาสามารถใช้ยาได้อย่างเหมาะสมและปลอดภัยมากขึ้น นอกจากนี้หน่วยงานที่ เกี่ยวข้องควรเพิ่มความเข้มงวดของการจ่ายยา isotretinoin ในร้านยาให้มากขึ้น |
| abstract: This special project was a qualitative study aimed to explore knowledge, perceived need, perceived risk, utilization characteristic and reasons of using oral isotretinoin. The total of 20 participants aged between 15 – 30 years old were selected by purposive sampling and snowball technique and were interviewed by semi-structured questionnaire. The result indicated that all of participants had received prior treatments before using oral isotretinoin, although some treatments were deviated from the guideline of prior treatment before using isotretinoin. Most participants received this medicine from physicians. A few (N=4) purchased it without prescription. One participant had received this drug from physician and then went to refill it at the drugstore without prescription. The reasons of using oral isotretinoin could divide into clinical reason due to severity of acne, and personal reasons concerning about personal beauty. Although most participants indicated that it was expensive, 17 of them accepted that it was worth using isotretinoin because of its efficacy. For perceive risk, all of them knew that it could cause dry lips. 10 and 5 participants knew about teratogenicity, and hepatic side effect, respectively. All of them had never checked liver function and plasma lipid level. From the study, pharmacists should play more roles in counseling patients about indication, side effects, and monitoring associated with isotretinoin use so that they can use this medicine effectively and safely. In addition, related organizations should enforce and tighten the regulation of dispensing isotretinoin in drugstore. |
