คำตอบ
Loading dose เป็นการให้ยาเพื่อให้ได้ระดับยาที่ต้องการในเลือดอย่างรวดเร็ว โดยสามารถบริหารยาได้ทั้งแบบ bolus หรือ short infusion โดยการให้ยาแบบ intravenous (IV) infusion หรือ IV drip หรือ intermittent infusion เป็นการบริหารยาโดยฉีดเข้าหลอดเลือดดำและใช้ระยะเวลา 15-90 นาที สำหรับ trastuzumab (Herceptin®) ควรให้แบบ IV infusion เท่านั้น ไม่ควรให้แบบ IV bolus โดยการ loading dose ใน cycle แรกของ trastuzumab เป็นการให้เพื่อที่จะได้ระดับความเข้มข้นของยาถึงช่วงเป้าหมาย (target range) อย่างรวดเร็วเช่นเดียวกัน เชื่อว่าการที่ได้ระดับความเข้มข้นของยาเทียบเท่ากับความเข้มข้นที่ steady state ในช่วงแรกจะทำให้เนื้องอกมีขนาดเล็กลง (tumor regression) ได้เร็วขึ้น (โดยเฉพาะเมื่อให้ร่วมกับยาเคมีบำบัด) รวมทั้งยังช่วยบรรเทาอาการจากโรคหรือมี eastern cooperative oncology group (ECOG) performance status (การประเมินความสามารถในการปฏิบัติกิจวัตรประจำวัน) ที่ดีขึ้น
