| บทคัดย่อ: โครงการพิเศษนี้มีวัตถุประสงค์เพื่อศึกษาการใช้ยา imipenem/cilastatin และ meropenem ของผู้ป่วยแผนกอายุรกรรม โรงพยาบาลบำรุงราษฎร์ โดยเก็บข้อมูลแบบย้อนหลังจากแฟ้มประวัติผู้ป่วย ตั้งแต่เดือนพฤษภาคมถึงกรกฎาคม พ.ศ. 2543 ผลการศึกษาพบว่า มีจำนวนผู้ป่วยที่ใช้ยาสองชนิดนี้ 36 คน อายุเฉลี่ย 54±22.12 ปี (มีช่วงอายุตั้งแต่ 18-91 ปี) เป็นผู้ป่วยชาย 20 คน ผู้ป่วยหญิง 16 คน มีการใช้ยา imipenem/cilastatin จำนวน 21 คน ใช้ยา meropenem 11 คน และใช้ยาทั้งสองชนิดในผู้ป่วยคนเดียวกัน จำนวน 4 คน โดยแต่ละครั้งที่มีการใช้ยาจะมีข้อบ่งใช้เดียว เหตุผลในการใช้ยาดังกล่าว คือ การป้องกันก่อนผ่าตัด 1 คน ใช้รักษาก่อนทราบผลความไวต่อเชื้อ 23 คน ใช้รักษาเมื่อทราบผลความไวเชื้อ 12 คน ในผู้ป่วยจำนวน 23 คนที่ใช้รักษาก่อนทราบผลความไวต่อเชื้อ ไม่มีการทดสอบความไวต่อเชื้อ 18 คน มีการทดสอบความไวต่อเชื้อ 4 คน และ 1 คนไม่ทราบว่ามีการทดสอบ ยา imipenem/cilastatin มีการใช้ในการรักษาปอบวมมากที่สุด 7 คน และยา meropenem ใช้ในการรักษาการติดเชื้อในกระแสเลือดมากที่สุด 4 คน และมีผู้ป่วย 2 คนที่ไม่ทราบข้อบ่งใช้ ในผู้ป่วยที่มีการใช้ยาimipenem/cilastatin 21 คน และ meropenem 11 คน มีค่ายาโดยเฉลี่ย 4,237 และ 7,171 บาทต่อวันต่อคน ตามลำดับ จากการศึกษานี้ ควรมีการประเมินการใช้ยาทั้งสองชนิดนี้ในด้านความเหมาะสมในการใช้ต่อไป |
| abstract: The purpose of this study was to find out the utilization of imipenem/cilastatin and meropenem of in-patients at medical ward, Bumrungrad Hospital. Patient data was collected retrospectively from medical records between May to July 2000.This study found that 36 patients used both drugs. The average age was 54 +-22.12 year olds (range 18 - 91). There were 20 male and 16 female. Imipenem/cilastatin was used in 21 patients, meropenem was used in 11 patients and both of them were used in 4 patients. Each course had only one indication. The reason for usage included surgical prophylaxis (1 patient), empirical treatment (23 patients), definitive treatment (12 patients). Among 23 patients with empirical treatment, eighteen patients were not documented for culture and sensitivity result after first dose, four patients were documented for culture and sensitivity result and 1 patient was not known culture and sensitivity result. Pneumonia was the leading indication for imipenem/cilastatin (7 patients) and sepsis was the leading indication for meropenem (4 patients), no known indication was identified in 2 patients. The average cost of imipenem/cilastatin (21 patients) was 4,237 Bahts/day/patient and meropenem (11 patients) was 7,171 Bahts/day/patient. From our study, drug utilization evaluation of imipenem/cilastatin and meropenem would be further study. |
