เนื่องจากยา mesalamine หรือ mesalazine (5-aminosalicylic acid) มีโครงสร้างทางเคมีที่คล้ายคลึงกับ aspirin จึงถูกจัดเป็นยากลุ่ม salicylate เช่นเดียวกัน ทำให้มีโอกาสที่จะเกิดการแพ้ข้ามกันได้ ดังนั้นหากทำได้ ควรหลีกเลี่ยง aspirin ในผู้ป่วยที่่มีประวัติแพ้ mesalamine โดยเฉพาะหากมีอาการแพ้รุนแรง อย่างไรก็ตาม อย่างไรก็ตามข้อมูลจากรายงานกรณีศึกษาพบว่าผู้ป่วยที่มีประวัติแพ้ aspirin ยังอาจสามารถใช้ mesalamine ได้ตามปกติโดยไม่เกิดอาการแพ้ หรืออาจต้องทำ desensitization ร่วมด้วย ดังนั้นหากมีความจำเป็นต้องใช้ aspirin ้เภสัชกรควรสอบทานแพทย์ถึงประวัติแพ้ยาและความเสี่ยงในการแพ้ข้ามกัน หากแพทย์ประเมินแล้วว่าประโยชน์จากยามีมากกว่าความเสี่ยง และผู้ป่วยยอมรับความเสี่ยงได้ อาจพิจารณาใช้ aspirin และติดตามอย่างใกล้ชิดต่อไป และเภสัชควรให้คำปรึกษาและแนะนำการปฏิบัติตัวที่เหมาะสมหากเกิดอาการแพ้ด้วยเช่นกัน
